Rifaximin is a non-systemic antibiotic that is primarily used to treat gastrointestinal infections and conditions. It is minimally absorbed into the bloodstream, making it effective locally in the gut with fewer systemic side effects. Below is a detailed overview of its mode of action, benefits, dosage, side effects, and indications.
Mode of Action
Antibacterial Activity:
Rifaximin is a rifamycin derivative that inhibits bacterial RNA synthesis by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase.
It is effective against gram-positive, gram-negative, aerobic, and anaerobic bacteria in the gastrointestinal tract.
Non-Systemic Action:
Rifaximin is poorly absorbed (<1%) in the gastrointestinal tract, allowing it to act locally in the gut without significant systemic exposure.
Benefits
Treatment of Traveler’s Diarrhea:
Effective against E. coli and other enteric pathogens causing diarrhea.
Reduction of Hepatic Encephalopathy (HE) Symptoms:
Reduces ammonia-producing bacteria in the gut, lowering ammonia levels and improving HE symptoms.
Irritable Bowel Syndrome with Diarrhea (IBS-D):
Alters gut microbiota and reduces symptoms like diarrhea, bloating, and abdominal pain.
Small Intestinal Bacterial Overgrowth (SIBO):
Reduces bacterial overgrowth in the small intestine, alleviating symptoms like bloating, gas, and diarrhea.
Minimal Resistance Development:
Due to its non-systemic nature, rifaximin has a lower risk of promoting bacterial resistance compared to systemic antibiotics.
Dosage
Traveler’s Diarrhea:
200 mg taken 3 times daily for 3 days.
Hepatic Encephalopathy:
550 mg taken twice daily for maintenance therapy.
Irritable Bowel Syndrome with Diarrhea (IBS-D):
550 mg taken 3 times daily for 14 days. This course can be repeated if symptoms recur.
Small Intestinal Bacterial Overgrowth (SIBO):
400–550 mg taken 2–3 times daily for 7–14 days.
Side Effects
Common Side Effects:
Bloating, flatulence, abdominal pain, nausea, and headache.
Serious Side Effects:
Rarely, may cause Clostridium difficile-associated diarrhea (due to disruption of normal gut flora).
Allergic reactions (e.g., rash, itching, swelling) are uncommon but possible.
